Non-custodial parent & emergencies.?

When a non-custodial parent has the child out of town on vacation... can that parent take the child to the emergency room if needed? My son has full legal & physical custody but is wondering if the child gets hurt can his ex take him for medical care if she needs to? Will a note from him help?
